Understanding Tooth Fillings and Dental Health

What Is a Tooth Filling? A tooth filling (or dental restoration) is a medical procedure to repair tooth structure that has been damaged by **decay (caries)** or **trauma**. The primary goal is to remove the decayed portion, clean the affected area, and then "fill" the cavity to restore the tooth's original shape, function, and integrity.

Who Can Benefit?

A filling is recommended for individuals experiencing:

  • **Dental Cavities (Caries):** The most common reason for a filling.
  • **Chipped or Broken Teeth:** Minor fractures can be repaired with filling material.
  • **Worn-Down Teeth:** Caused by grinding (bruxism) or improper biting.
  • **Replacing Old Fillings:** Replacing old, leaking, or failed amalgam (silver) fillings.
  • **Tooth Gaps:** Can be used to close small gaps between teeth (dental bonding).

Why It's Important

Treating decay is a medical necessity. An untreated cavity can spread, leading to:

  • **Stops Decay:** Prevents bacteria from destroying more of the tooth.
  • **Prevents Infection:** Stops decay from reaching the nerve (pulp), which causes abscesses.
  • **Avoids Tooth Loss:** Saves the tooth from extraction or needing a root canal.
  • **Restores Function:** Allows you to chew properly and without pain.

Types of Modern Dental Fillings

Treatment TypeMaterialBest ForAestheticsDurability (Typical)
Composite ResinTooth-colored plastic and glass mixture.Small to medium cavities (front & back teeth).Excellent (matched to your tooth shade).5-10 years
Ceramic (Inlay/Onlay)Medical-grade porcelain, milled by CEREC®.Large cavities, broken cusps, replacing large amalgam fillings.Superior (translucent, stain-resistant).10-20+ years
Glass IonomerAcrylic and glass powder.Small non-biting areas (near gumline) or temporary fillings.Fair (Opaque white).~5 years (Releases fluoride)

Our Amalgam-Free Philosophy: Amir Beladi Polyclinic is a **mercury-free practice**. We do not place amalgam (silver) fillings. We exclusively use modern, biocompatible, and aesthetic materials like composite resin and ceramic that chemically bond to your tooth for a stronger, safer, and more natural-looking result.

The Procedure Timeline & Experience

1

Diagnosis & Anesthesia

10-15 Minutes

  • Review of digital X-ray to confirm decay.
  • Color-matching the tooth (for composite/ceramic).
  • Application of topical numbing gel.
  • Administration of local anesthetic (the "injection").
2

Cavity Preparation

15-20 Minutes

  • Isolating the tooth (often with a rubber dam).
  • Gentle removal of all decay using a high-speed drill.
  • Refining the cavity shape with a slow-speed drill.
  • Thorough cleaning and disinfection of the cavity.
3

Material Placement

15-30 Minutes

  • **For Composite:** Etching the tooth, applying bonding agent, placing the composite in layers, and "curing" each layer with a blue light.
  • **For Ceramic:** Taking a digital 3D scan (CEREC®) or impression.
4

Polishing & Bite Check

10 Minutes

  • Shaping the filling to match natural tooth anatomy.
  • Polishing the restoration to a high shine.
  • Checking the "bite" with articulating paper.
  • Making final adjustments for perfect comfort.

Did You Know? (CEREC® Technology): For large ceramic inlays and onlays, we use **CEREC® technology**. This allows us to digitally design, mill, and place your permanent ceramic restoration in a **single visit**, eliminating the need for temporary fillings or a second appointment.

Pre-Treatment Preparation

Before Your Appointment

  • Ensure you have had a recent dental check-up and cleaning (within 6 months).
  • No special preparation is typically needed for a standard filling.

The Day of Your Filling

  • **Eat a normal meal** a few hours before; you will be numb and unable to eat for 2-4 hours after.
  • **Brush and floss** thoroughly before arriving.

What to Discuss

  • Inform your dentist of **all medications** you are taking.
  • Mention any **allergies** (especially to latex or anesthetics).
  • Communicate any **dental anxiety** you may have.
A Note on Dental Anxiety:

Feeling anxious about dental procedures is extremely common. We are here to help. Please let our team know, as we offer several comfort options, from simple distractions to conscious sedation, to ensure your experience is as calm and pain-free as possible.

Aftercare & Recovery Timeline

First 2-4 Hours (Numbness)

Your lip, tongue, and cheek will be numb.

  • DO NOT eat or chew. You can easily bite your cheek or tongue.
  • AVOID hot drinks (coffee/tea) to prevent burns.
  • Cool liquids are acceptable, but drink carefully.

First 24-48 Hours (Sensitivity)

Mild sensitivity to hot, cold, or air is normal.

  • You can eat normally once numbness fades.
  • Try to chew on the opposite side for the first day.
  • AVOID extremely hard, sticky, or chewy foods (e.g., toffee, ice).

Long-Term Care (Weeks 2+)

Sensitivity should fade. The filling is fully functional.

  • **Brush and floss** normally. It is critical to keep the filling margins clean.
  • Use a **desensitizing toothpaste** if sensitivity lingers.
  • Attend your regular **6-month check-ups** so we can monitor the filling.

Warning Signs: Contact Us Immediately

Normal healing involves mild, fading sensitivity. Call us if you experience:

  • ⚠️ **A "High" Bite:** The filling feels tall when you bite. (Needs simple adjustment).
  • ⚠️ **Sharp, lingering pain** to hot or cold (lasting > 10 seconds).
  • ⚠️ **Spontaneous pain** or a persistent "toothache" (throbbing).
  • ⚠️ A visible **crack or fracture** in the new filling.

Safety, Side Effects, and Clinic Protocols

Common Side Effects

  • Sensitivity to Cold/Hot: ~30-50%. Usually mild and resolves in 1-2 weeks.
  • Bite Discomfort: ~10%. The bite may feel "different." If it feels "high," it requires a simple adjustment.
  • Gum Irritation: ~20%. Minor soreness near the injection site or from the dental clamp, lasting 1-2 days.

Rare Complications

  • ⚠️ Allergic Reaction: Extremely rare, but possible, to anesthetic or bonding agents.
  • ⚠️ Irreversible Pulpitis: In very deep cavities, the nerve may become irreversibly inflamed, requiring a root canal.
  • ⚠️ Filling Fracture/Debonding: Can occur from trauma or biting on extremely hard objects.

Amir Beladi Safety Protocols

  • Rubber Dam Isolation: We use a rubber dam whenever possible. This isolates the tooth, preventing saliva contamination for a stronger bond and protecting you from swallowing debris.
  • High-Magnification Loupes: Our dentists use high-magnification lenses to ensure 100% of the decay is removed while preserving the maximum amount of healthy tooth structure.
  • Advanced Bonding Technique: We use multi-step bonding systems (not all-in-one) which are clinically proven to create a more durable, sealed, and long-lasting restoration.

Results & Realistic Expectations

Realistic Timeline

  • Immediate: Full function and aesthetics are restored. You will leave the clinic with a completed, polished filling.
  • 1-2 Weeks: Any minor sensitivity to temperature should fully resolve.
  • Long-Term: The filling should feel and function exactly like your natural tooth.

Maintenance Requirements

  • Hygiene: Fillings require the same care as natural teeth: brushing twice daily and flossing once daily.
  • Habits: Avoid biting non-food items (ice, pens, fingernails) which can fracture both fillings and natural teeth.
  • Grinding (Bruxism): If you grind your teeth, a **night guard** is essential to protect your fillings and teeth from excessive force.

Success Rates

Modern dental restorations are highly successful. Clinical data shows that when placed with proper technique (like rubber dam isolation and advanced bonding), composite fillings have a **>95% success rate over 5 years**. Ceramic inlays/onlays are even more durable, with success rates often exceeding **95% over 10 years**. The most common cause of failure is new decay forming *around* the filling, which is preventable with good hygiene.

Personalized Treatment Investment

Because every tooth and cavity is different, a transparent investment plan can only be provided after a detailed clinical assessment and diagnostic X-rays.

How We Determine Your Investment:

  • Size & Complexity: A filling is priced by the number of tooth surfaces involved (e.g., 1-surface vs. 3-surface).
  • Material Used: Composite resin has a different cost from a lab-milled ceramic inlay/onlay.
  • Technology Used: Services like same-day CEREC® milling are factored into the cost of ceramic restorations.

We will provide a clear, itemized quote during your consultation. We accept most major insurance plans and offer flexible payment options.

Frequently Asked Questions

Does getting a filling hurt?
No. The procedure is performed under **local anesthetic**. We apply a strong topical gel to numb the gum first, so even the injection is barely felt. You will feel pressure and vibration from the drill, but no pain.
How long does a tooth filling appointment take?
A single, straightforward composite filling typically takes **30 to 60 minutes** from start to finish. A same-day CEREC® ceramic restoration may take **90 to 120 minutes** to allow for digital scanning and in-house milling.
How long until I can eat after a filling?
You must wait until the **numbness completely wears off**, which is usually **2 to 4 hours**. Eating while numb poses a high risk of severely biting your tongue or cheek. Once feeling returns, you can eat, though we advise starting with softer foods.
Is sensitivity after a filling normal?
**Yes, this is the most common side effect.** Mild sensitivity to cold, hot, or sweet things is normal for a few days to a few weeks, especially with deeper fillings. It occurs because the tooth's nerve has been slightly irritated. It should fade gradually.
Why do I need a filling if my tooth doesn't hurt?
Decay often does not cause pain until it is very large and close to the nerve. We use X-rays and clinical exams to find cavities when they are **small and easy to treat**. Waiting for pain often means the decay is advanced, leading to a much higher chance of needing a root canal or extraction.
How long do composite (white) fillings last?
Durability depends on the filling's size, location (biting-force), and your hygiene. On average, a composite filling lasts **5 to 10 years**. A ceramic inlay/onlay is much stronger and can last **10 to 20+ years**.
What is an inlay or onlay?
An **inlay** or **onlay** is a type of ceramic filling. It is used when the cavity is too large for a simple composite filling but not large enough to require a full crown. An **inlay** fits *within* the cusps (bumps) of the tooth, while an **onlay** covers one or more cusps. They are stronger and more durable than composite.
My old silver filling is fine. Why should I replace it?
We don't replace fillings just because they are amalgam (silver). However, old amalgam fillings can **leak**, **corrode**, or cause **micro-fractures** in the tooth over time. We only recommend replacement if we see signs of new decay underneath, leaking margins, or fractures on an X-ray.
